Head southwest from Bakbattahl until you reach the Wyrmsblood Forest to ascertain where Medusa is lurking. Once you heed warnings about the dangers ahead, it means you're on the right track. The closer you get to Medusa's cave in the Caliginous Depths, the more petrified statues you'll encounter. Your party will deal greater damage when equipped with slashing weapons.

Medusa’s petrifying gaze can turn you and your party to stone in an instant, and claiming her head isn't simply a matter of slicing it off after the battle. Every move is crucial to your survival, but much like other formidable enemies, Medusa is not without weak points. Channel your attack towards her head and neck the most to exploit her weakness.

You might want to bring a Panacea along as it effectively cures most ailments. Ensure you're also equipped with the Ring of Motility, which negates her ability to petrify you. If possible, employ ranged weapons to strike her weak points accurately, and you can also capitalize on the collapsing pillars as cover to dodge her lethal gaze.

For close range, climbing on her and striking her neck repeatedly also does the trick. Once she's stunned, deliver a forceful blow to decapitate her. If you manage to defeat her, loot her body to claim the Medusan Spellbow, the Hood of Nonentity, Granite Medusa Bone, and retrieve her head. Medusa’s head decays over time, eventually withering and losing its petrifying power.

You can store it in an inn's storage when not in use to slow the pace of decay. Since the head is in your possession, you can use its petrifying power against enemies as needed, even against Medusa herself. Medusa might respawn after a while, but you can use her own head to petrify her during the battle and unlock achievements such as “Off with Its Head!” and “An Eye for an Eye.”
