The Organ Transport Box is one of those classic Resident Evil progression gates that looks straightforward but won’t budge unless you’ve followed the intended chain of puzzles inside the Rhodes Hill Care Center.
You’ll first find it in the Garage, tucked inside the vehicle at the back of the room while playing as Grace.
By then, it becomes obvious the box belongs to the organless body in the Examination Room, but grabbing it alone won’t work since the back of the case is secured by a mechanical control panel.
How Do You Get the Instructions to Unlock It?
While you can experiment with the switches manually, the proper way to learn the solution is by progressing through the East Wing on the second floor.

After acquiring Level 1 clearance, you’ll eventually reach the Lead Researcher’s Office. A locked puzzle box sits inside, and a nearby note points you toward retrieving Corrosive from the Conference Room to melt the padlock.
Once you’ve dealt with the threats in that area and solved the symbol-based puzzle box (which also ties into the larger Moon, Sun, and Star door progression), you’ll receive a key item along with a file that specifically explains how to unlock the Organ Transport Box.
That note shows the exact order of inputs required on the rear control panel.
How Do You Actually Open the Organ Transport Box?
To actually open the case, inspect the Organ Transport Box in your inventory and rotate it to reveal the back panel.

If you’ve already moved anything, use the reset function first so the switches return to default.
Begin by turning the main dial from AUTO to HAND. Next, raise the vertical slider on the right side until it sits in the middle position.
After that, adjust the numbered toggle switches so that positions 2 and 5 are activated according to the diagram shown in the file. Once those are in place, turn the main dial back from HAND to AUTO.
If everything was entered properly, the case will unlock immediately. Inside, you’ll find the Artificial Lungs. This is only one half of the larger Organless Corpse puzzle, however.
To fully complete that objective, you must also obtain the Artificial Heart, which is locked behind another gated room in the Care Center that becomes accessible after further progression and use of the Wrench.
When you return to the Examination Room and insert the two artificial organs, the puzzle completes, triggering the body’s reanimation and forcing you to deal with it before progressing.
