This isn’t something that happens naturally very often, so you’ll have to deliberately manipulate enemy behavior.

Your best bet is to do it while you're at the care center, specifically by luring the ones who shuffle around with an IV drip stand.

These IV zombies have more exaggerated attack animations, which makes it easier to redirect their aggression toward another undead enemy instead of yourself.

How To Unlock The Internal Dispute Resident Evil Requiem Trophy?

The best place to set this up is near the Records Room in the hospital section.

Just before entering that area, there’s an office where you can pick up an empty bottle.

This item is crucial because it allows you to create a noise distraction without firing a weapon or putting yourself in direct danger.

After grabbing the bottle, proceed quietly into the Records Room zone and scout the area carefully. Your goal is to locate an IV zombie and at least one regular zombie positioned close enough that an attack could realistically connect between them.

Once you’ve identified both targets, crouch and move slowly to adjust their spacing if needed. You want them standing within arm’s reach of each other.

When they’re properly aligned, throw the empty bottle in a way that provokes the IV zombie’s aggression while keeping the second zombie directly in its path.

The noise should trigger the IV zombie to swing its weapon or lunge forward. As long as your positioning is right, the enemy’s attack will hit a fellow zombie and trigger the trophy.

Be prepared for some trial and error. If the IV zombie focuses on you instead, reposition and try again. Avoid standing too close when throwing the bottle, as that can pull all nearby enemies toward you at once.

Patience and spacing are the key factors here. Once the IV zombie successfully lands a hit on another zombie, the Internal Dispute trophy will unlock.
