The Makarov doesn't feature in the Season 4 Battle Pass which means you need to complete a seasonal challenge in order to obtain it. The challenge you need to complete is known as Sidearm Scuffle where you can also unlock a Silver Crate Coupon and a blueprint for the .50 GS.

To unlock the pistol, complete all of the tasks by eliminating players with pistols. Once complete, you can use the Makarov in a loadout and customise it using the Gunsmith.
